In the absence of a centralized federal database listing instances of misconduct, the Project On Government Oversight (POGO) is providing such data. Synopsis: It was reported that the Wales Health and Safety Executive (HSE) fined BAE Systems Land Systems (Munitions and Ordinance) Ltd. £50,000 (USD$ 88,954) and ordered it to pay £15,000 (USD$ 26,692) in costs and £5,000 (USD$ 8,897) in compensation for health and safety breaches for an incident in which an employee was injured handling hazardous material at its site in Glascoed, Monmouthshire in August 2004. HSE found that the disposal procedures did not comply with BAE's own approved control procedures, and that management and supervision of the procedure was inadequate.
