March 2012 Memphis Refinery Fire – TOSHA Investigation

The Tennessee Occupational Safety and Health Administration (TOSHA) proposed fining Valero Refining Company-Tennessee, LLC $63,000 for a March 2012 fire at its Memphis refinery that killed one worker and injured two others. TOSHA found 16 serious violations.
