Pembroke Refinery Safety Breach
In March 2012, a worker was seriously injured when he fell from an access tower walkway at Valero Energy UK’s Pembroke refinery in Wales. The U.K. Health and Safety Executive (HSE) found the incident was caused by multiple workplace safety failings. Valero pleaded guilty to a single charge of breaching Section 2(1) of the Health and Safety at Work etc Act 1974 and was fined £400,000 (US$529,206) and ordered to pay costs of £60,614 (US$80,193).
