Cape Canaveral Air Force Station Safety Violation

In July 2015, a United Launch Alliance technician lost two fingers while working on a launch pad at Cape Canaveral Air Force Station. After conducting an investigation, the Occupational Safety and Health Administration (OSHA) cited ULA for a serious violation for failing to provide machine guarding and assessed a $5,000 penalty.
