Hanford WTP Worker Health and Safety Violations – November 2016
The U.S. Department of Energy (DOE) issued a Preliminary Notice of Violation (PNOV) to Bechtel National for violations of worker safety and health requirements at the Hanford Waste Treatment and Immobilization Plant (WTP). The violations concern an event at the WTP in November 2016, when the release of a pressurized stream of water caused by a separated pipe injured a worker. The PNOV cites three Severity Level I violations and two Severity Level II violations in the areas of management responsibilities, hazard identification and assessment, hazard prevention and abatement, training and information, and record keeping. In lieu of a civil penalty, DOE withheld $556,500 in contract fees for significant failures in safety and health performance, including the matters associated with the PNOV.
