Investigation of 2018 Worker Injury Incidents

In April 2019, the U.S. Department of Energy (DOE) Office of Enterprise Assessments’ Office of Enforcement notified Triad National Security LLC and subcontractor Cross Connection of its intent to investigate potential deficiencies in the implementation of DOE worker safety and health program requirements at the Los Alamos National Laboratory. The investigation was prompted in part by an incident in December 2018 when a Cross Connection worker was seriously injured after he was struck by a falling lifting hook attachment from a skid steer loader at the lab. The scope of the investigation will include Triad’s involvement in implementing and sustaining corrective actions associated with a July 2018 Cross Connection worker injury incident.
