Inspection of San Diego Facility

In June 2015, the U.S. Nuclear Regulatory Commission (NRC) conducted an inspection of the Thermo Fisher Scientific (TFS) facility in San Diego, California in order to evaluate TFS’s implementation of quality activities associated with the fabrication and testing of safety-related excore neutron flux monitoring systems supplied to U.S. operating reactor plants. The NRC determined that TFS failed to notify the NRC that a defect existed in a Power Range Detector that was shipped to Palisades Nuclear Power Plant. The NRC also found that TFS failed to ensure that K-type thermocouple measuring devices were identified with a serial number and calibrated at periodic intervals to maintain accuracy within necessary limits.
